What is FREMEC?

FREMEC (Frequent Traveller Medical Card) is a document that facilitates the boarding of frequent passengers who have special medical conditions, of a permanent, non-severe, stable nature, and without other comorbidities.

 

Examples of medical conditions eligible for FREMEC include:

 

  • Permanent visual impairment - it is necessary to attach an ophthalmological report informing visual acuity issued within a maximum of 12 months;
  • Permanent hearing impairment - it is necessary to attach an audiometric evaluation carried out within a maximum of 12 months;
  • Autism Spectrum Disorder (ASD) - it is necessary to attach a diagnostic report with a detailed description of the condition, issued by the attending physician within a maximum of 12 months;
  • Tetraplegia/Hemiplegia

 

After the medical area evaluates the information contained in the forms/reports, if approved, the FREMEC card will be sent to the passenger and will be valid for a maximum of 1 year (at medical discretion). During this period, in possession of the card, the passenger will no longer need to present the MEDIF (Medical Information Form) but only the FREMEC, identification document, and ticket proof, thus ensuring greater agility in their service and boarding.

 

In the case of the passenger presenting an acute illness, unstable chronic disease, infectious diseases, comorbidities, or pre and post-operative condition, the MEDIF medical form must be completed.

 

The following medical conditions do not require medical authorization (MEDIF or FREMEC), as long as they are controlled and stable:

 

  • Diabetes Mellitus;
  • High blood pressure or high cholesterol;
  • Arthritis;
  • Artificial limbs.

 

We emphasize that the FREMEC card cannot be issued to passengers who require oxygen supplementation for use on board.

 

Important: Passengers who already have the FREMEC card must immediately inform AZUL whenever their medical condition changes or worsens.

 

The FREMEC form is available for download at the end of this page, and customers must ensure its complete filling and signature by the passenger's personal physician before booking the ticket. This form must be sent to AZUL at least 7 business days before the flight through our website. The medical documents and FREMEC will be evaluated by AZUL's medical professionals, specialized in aviation medicine, with a response time of up to 7 business days. AZUL's medical department may request additional information if necessary.

 

It is worth remembering that passengers who need special assistance but are not eligible for FREMEC will not be prevented from boarding but must proceed with sending the MEDIF form (following the analysis period from a new submission). We remind you that the FREMEC form (and report when necessary) must contain the signature and CRM of the passenger's personal physician. This medical form will not be accepted if it is incomplete or signed by psychologists, physiotherapists, and other professionals different from doctors with a valid CRM. The filling by the doctor must be done a maximum of 30 days before submission. Forms dated more than 30 days will not be accepted.

 

For the evaluation of FREMEC from another airline, it will be necessary to send the FREMEC medical forms/reports, along with the authorization from the other airline, which must include the number/validity so that the validity of the same document can be analyzed by AZUL's medical team.

 

AZUL's medical department will respond in writing to the passenger's requests for a companion within up to 7 (seven) business days. If the FREMEC request is approved with the right to a companion, a fee equal to or less than 20% (twenty percent) of the airfare purchased by PNAE will be charged for each additional seat necessary for the service. This discount does not apply to fees owed to airport operators and/or government entities, as well as to additional services offered by AZUL, except those described in items I and II of §3, of article 8 of the aforementioned Resolution.

 

It is important to emphasize that the discount for the companion will only be granted if the need for a companion is verified and approved by AZUL's medical department, under the terms of ANAC resolution 280 (when due to mental or intellectual impairment, the passenger cannot understand flight safety instructions; or cannot attend to their physiological needs without assistance). When a companion is indicated, a responsible adult in full physical and clinical condition, capable and able to meet the passenger's needs, must be indicated.

 

AZUL also emphasizes that the data requested in the medical form arise from legal requirements and are essential to enable the air transport service. Thus, FREMEC data are treated as sensitive, analyzed solely by specialized medical professionals, and safeguarded according to medical confidentiality prerogatives and passenger privacy.

How It Works

Once a passenger has their card, they will not need to present their MEDIF (Medical Information Form) any longer; just their FREMEC, an identification document, and a plane ticket confirmation. This will ensure a speedy service and boarding.

FREMEC Request Examples

Below are examples of assistance to people with reduced mobility due to a permanent, stable disability:

 

  • Visual impairment - you must attach an ophthalmological report informing the passenger’s visual acuity carried out in the last 12 months;
  • Hearing impairment - you must attach an audiometric report carried out in the last 12 months;
  • Autism Spectrum Disorder (ASD) - you must attach a report from your doctor, informing the absence of comorbidities and the stable nature of presentation, carried out in the last 12 months.

Filling out the FREMEC

Answer all questions correctly, with legible handwriting. Put an X in the Yes or No options. After that, all copies of the form must be signed.

Sending the form

Fill out Parts 2, 3, 4 and 5 of the FREMEC and follow the sending instructions below. Please bear in mind that the form is valid for 30 days after AZUL’s medical authorization for domestic and international flights.

Buy your ticket via Customer Service (4003-1118) and inform your FREMEC number when you book it;

At boarding, all you need to do is present your documents. A doctor’s note or an authorization is not required.

Attention to the send date deadline

This form must be sent to AZUL at least 7 working days before the flight via our website. The medical documents and FREMEC will be analyzed by AZUL’s medical professionals specialized in aviation medicine, within 07 working days.

Attention to the validity date

If approved, the FREMEC is valid for 01 year, counting from AZUL’s answer. Only forms filled out up to 30 days before their send-in date will be valid for analysis. After 30 days, a new form must be sent for analysis. It may be issued either by Azul or another airline company, provided that the passenger’s health conditions are stable.

Employee signature

The FREMEC (and a medical report when necessary) must contain a signature and the passenger’s personal doctor’s CRM (this medical form shall not be accepted if it is signed by psychologists, physical therapists, and other non-medical professionals without a valid CRM).
