In some cases, if you or the passenger for whom you are buying a ticket is not in perfect health conditions, it is mandatory to fill out a MEDIF (Medical Information Form) Form. This happens when the passenger:

  • Is suffering from an illness or disability that affects their health or well-being, or could possibly affect that of the other passengers and crew;
  • Has recently undergone surgery;
  • Has a health condition considered to be unstable;
  • Poses a risk to the safety or timeliness of the flight;
  • Needs attention or medical monitoring and/or special equipment during the flight.

The MEDIF form is provided by Azul for customers for them to complete and have signed by the passenger’s personal physician before booking the airline ticket. The form should be sent to Azul at least 72 hours before the flight’s departure, by e-mail or fax. The medical document and the MEDIF form will be evaluated by Azul’s medical service, which specializes in aviation medicine, with a maximum response time of 48 (forty-eight) hours useful. It is worth bearing in mind that the air transport of customers in the circumstances described above is subject to prior clearance by Azul’s medical department, based on the information contained on the form. If Azul’s requirements are not met, the passenger’s transport may be refused.

Passengers with disabilities or reduced mobility must be accompanied by a person over 18 years of age, if due to mental or intellectual disability, they are unable to understand the flight safety instructions or cannot meet their physiological needs without assistance. In such cases, Azul will require the presence of a person chosen by the passenger to accompany them, with a discount of 80% applying to the booking rate paid in Brazilian Reais (this does not include bookings issued in air mile points or points+reais) of the passenger requiring assistance. Azul will provide written response within 48 hours to customer requests for a person to accompany them. Azul does not cater to requests for stretcher transport or medical oxygen use (portable or pressurized oxygen portable cylinders) on board its aircraft (ATR, Embraer - EJet and Airbus) due to lack of aircraft certification for this equipment, in addition to operational safety issues. The company is waiting for Brazilian standards to be defined on the use of portable oxygen breathing equipment.

The MEDIF form needs to be signed by the passenger’s personal physician (this form will not be accepted if it has been signed by psychologists, physiotherapists or other healthcare professionals other than physicians with a valid CRM [Conselho Regional de Medicina (Regional Board of Medicine)] number).
